HNO Community Health Networks

What is a Community Health Network?

A Community Health Network is a coalition of individuals and organizations who are committed to improving the health physical, emotional, mental, financial, and spiritual health of every resident within a defined neighborhood.

During 2001, three Community Health Networks were initiated:

• Carrollton United
• Bienville/Conti/Tulane Collaboration
• St. Bernard Coalition
